Need to create linear 4th axis

I've built a fourth axis for my CNC table to feed a 3D print extruder. The problem is I need it to be a linear axis not an angular one. I've tried modifying the ini file in the TRAJ section and it runs that way but the 4th axis still goes by degrees and doesn't feed anywhere near what I need.
There must be a way to do this but I don't know how. I've examined all the files but can't seem to see the magic bullet.
    Any ideas?

A linear axis parallel to X would be U. If I recall you would set it up in the INI as 7 axiis and list XYZU as the designations, without designating A B or C. If you have more questions I can copy a small part of my INI as an example.

Thanks Ed. I got this working with the great help at LinucCNC.org. Pretty much what you said.
    Thanks again for the reply,
