It requires live tooling. It can be done either via a sycnronised serveo, or some machines use a mechanical drive from the spindle.
One key point to remember is it doesn't produce perfect flats. If you search for Horn polygon milling tools, they have a catalogue which lists the possible/recommended shapes possible.
Check out eurotechs if you're doing production.
