Couple things to experiment with. Change min PWM to 1%. Change the Step Low Active to disabled. Change Step and Dir ports to 1 like the stepper motors.

That seems to work well for me. Once you get up close to the maximum listed, it's easy to stall the motor unless you slow down the feed rate quite a bit. I can't way to see how others weigh in though.
Try swapping that motor to another known good drive and cable. If it works, it could be the driver or something wrong with your cabling.
Clint you answered my question as you are not using a G540. Looks like Don's solution may be perfect for you. I might even switch my spindle control out to this since it sounds like it does closed...
